understanding the Samaritan Membership
our data indicates that individuals who engage with a Samaritan Membership for a duration of 6 to 12 months significantly accelerate their journey towards stable housing. the best outcomes are achieved when a Member meets with their care team partner—this could be a case manager, patient navigator, or care coordinator—at least once a month.
engagement and support
regular engagement is key to success. if a Member becomes unengaged, defined as having no contact for three months, you and your team will have the option to decide whether to exit them from the program. this ensures that we can focus our resources on those who are actively participating in their journey.
Membership extensions
if one of your Members requires a membership extension, please reach out to our team at support@samaritan.city. we will review extension requests on a case-by-case basis to ensure that we can provide the necessary support.
