# How to Find/Change Your Primary Care Provider (PCP) in California
## Who is my PCP? Your PCP is your main doctor for checkups, everyday health concerns, and referrals to specialists.
We know it can be easy to lose track of your doctor's information. Maybe it's been so long since you've been to the doctor you're not sure what to do or where to go. Or maybe you don't feel respected by your provider and don't know how to ask for a new one. Whichever it is, we've got you covered!
**Use the simple guide/script below.**
1. Call your Health Insurance at the following number and have your Medicaid ID ready (on your Medicaid card). If you don't have your card/are unsure of your ID, you can ask the representative to look you up by name and date of birth instead:
**Molina** - 888-665-4621
**Anthem Blue Cross** - 800-407-4627
**HealthNet** - 800-675-6110
**Alameda Alliance** - 877-932-2738
Not sure which plan you have? You can call the Medi-Cal helpline at 1-800-541-5555 and ask for help.
2. Use this script to find out who your PCP is:
"Hi! My name is \_\_\_\_\_. Can you help me figure out who my primary care doctor is? I would like the name, phone number, and address please."
Once you have the phone number; call and ask to set up an appointment with your doctor.
\*If transportation assistance is needed to make it to your appointment, please ask your health plan what transportation benefits you have available.
3. Use this script to change your PCP:
"Hi! My name is \_\_\_\_. I currently have a primary care doctor but I would like to request a new doctor instead \[at the same/a different clinic\]. Can you help me with this process?"
For switching, you can either be *assigned* a new doctor, or *pick* *your own*. If you want to pick your own, ask the representative: "Can you send me a list of doctors near me who are accepting new patients?" and review before calling back with the provider name you'd like to be switched to.
*TIP: If asked the reason for change, you can simply say it's a personal preference.*
It will likely take a few days for your PCP to be officially changed. In the meantime, you can still see your current doctor if needed. Once you're ready to set up an appointment, call the phone number provided by the representative and ask to set up an appointment. You will likely be considered a new patient, unless you switched doctors at the same clinic.
